*Please fill up Non-Disclosure Agreement (e-signed) before submitting IP request form online. Steps to fill up NDA:
- Download Non-Disclosure Aggreement_pHNiNE.doc file.
- Fill up the name of the individual/ Company/ Organization with complete registered address in the first page.
- Put your signature in the last page with the name, designation and date.
- Convert the doc file into *.pdf (If you have e-signature, could put the e-sign at the last page of the *.pdf file)
- Upload the file with other information as mentioned in the right column.
